feral cat with heart outside its body

a family of feral cats live on my patio. I have not been able to locate any program to spay/neuter them. today, the male (a large siamese) came on the porch and he has a body part outside of his body. it looks like the heart. it is in the same area as the heart. could this have happened due to a fight or would a human have been responsible? it looks awful. I want to help him but he no longer trusts humans. He had just gotten to the point where i could touch him and he would rub against me. Can you help
First, let me apologize for taking a while to answer you. I am currently traveling in Europe and internet connections are unreliable and hard to find in some areas. My many reference books are not available here so I'll try to guide you to some help.
I don't think the feral cat has its heart outside its body...it sounds like it is a horrific injury that has some growth, swelling, or something like that with a heart-like shape. I think its wonderful that you are helping feral cats. I've adopted a few over the years and with patience, they are loving and grateful creatures.
